the Independent Older Adult Through COLLAGE
COLLAGE members are improving the lives of their residents through use of COLLAGE assessments and access to the national data repository. Members are better able to develop resident-focused service plans; tailor health and wellness programs and services to their residents' needs; make informed data-based choices about risk and level of care decisions; implement preventive programs and services targeted to specific campus-wide needs; gather outcomes information to evaluate the effectiveness of their programs and services; and help residents age in place more gracefully.
COLLAGE Consortium members
will also have the opportunity to participate in a number of work
groups that will focus on refining existing assessment instruments,
developing additional assessment instruments, and creating interventions
to address problems faced by residents at many communities. It
is through these work groups that Consortium members can come
together to recommend standards, recognize their collective strengths
and challenges, and take steps to improve and broaden the services
they provide.
