Peer Group for Managers and Administrators with a special focus on reports
WHO: Managers and administrators responsible for COLLAGE implementation.
There is a wealth of information available from the COLLAGE assessments to develop meaningful
and targeted health/wellness programs and services for your residents and community. The
focus will be on how your organization can harvest information, using Excel, to inform better
decisions around developing preventive programs, allocating resources and managing risk. This
session is a basic introduction to learning about the types of reports that could be generated for
better outcomes. The group will meet every other month. |

COLLAGE Report Generation: Helping your staff make the most out of resident
health and wellness data
WHO: IT and other staff familiar with Excel/Access who may be responsible for COLLAGE report
generation
COLLAGE assessments contain a wealth of information to develop targeted health/wellness
programs and services. For example, knowing which residents/clients residing on a particular
campus are coded as "depressed" and are on specific medications. Or, knowing which residents
with a diagnosis of coronary heart disease are coded as needing help in specific areas of activities
of daily living would be advantageous to managing risk more effectively. This bi-monthy session is
for those who have a familiarity with Excel/Access and will be responsible for harvesting data and
generating reports.

This forum invites you to share your experience with COLLAGE -- what is working and what is not.
In advance of the session, we’ll ask for specific ideas that you’d like to share that might provide
some robust discussion material for the group. As COLLAGE develops and becomes more sophisticated, there is an increasing need to find venues for member dialogue. |

Making the Most of the COLLAGE Conversation: Are You Getting to the Heart of the Matter?
The COLLAGE assessment conversation covers a wide range of domains and some of them may be considered highly personal such as depression, pain, ADL/IADL, memory, etc. If any area is glossed over or avoided for fear of raising anxiety, communities may miss out on important resident information. Learn some of the ways members are getting to the heart of the matter with thoughtful questions and attention to conversation details. |
